Atsena Therapeutics reported preliminary findings on 7 October 2026 from Parts A and B of the LIGHTHOUSE trial of ATSN-201, an investigational gene therapy for X-linked retinoschisis (XLRS). The company reported improvements in retinal structure and selected functional measures, including microperimetry gains in treated patients that exceeded a level identified by the FDA as potentially supportive of approval. The data come from small, interim cohorts, and the pivotal Part C cohort is still enrolling.

Preliminary LIGHTHOUSE Findings

  • Data reported: Preliminary results from LIGHTHOUSE Parts A and B
  • Structure and function: Improvements in retinal structure and selected functional measures
  • Microperimetry: Improvements exceeded a level identified by the FDA as potentially supportive of regulatory approval, according to Atsena
  • Visual acuity: Three patients treated at the pivotal dose achieved 20/40 or better
  • Durability: Benefit maintained in the earliest-treated patient through three years of follow-up

These observations involve small patient groups. The relevant standard is whether the pivotal portion produces a reproducible, prespecified difference across the treated population.

X-Linked Retinoschisis

XLRS is an inherited retinal disorder that primarily affects males. It is generally caused by mutations in the RS1 gene, which encodes retinoschisin, a protein that maintains retinal structural integrity. Patients may experience reduced central vision, retinal layer separation and progressive visual impairment.

The disease can fluctuate on imaging, and central retinal cavities do not map precisely to a patient’s ability to see. Microperimetry, acuity and patient-relevant function therefore need to be assessed alongside optical coherence tomography (OCT). Investigators also need to account for baseline severity, age and the possibility that eyes with longstanding damage have less capacity to respond.

How ATSN-201 Works

ATSN-201 is designed to deliver a functional RS1 gene to retinal cells through subretinal administration, using a gene-delivery platform developed to improve retinal distribution. By addressing the underlying genetic defect, the therapy aims to support retinal structure and visual function.

Surgery and Safety

Subretinal delivery requires specialist surgery. The procedure carries risks independent of the vector, and immune response or inflammation may affect the eye after delivery. The full safety record should separate surgical events from treatment-related events and include longer follow-up, since a favorable mean outcome can mask meaningful harm in a small subgroup.

Next Steps

  • Pivotal cohort: LIGHTHOUSE Part C, in progress
  • Enrollment completion: Expected in the first quarter of 2027
  • Topline results: Anticipated in the first half of 2028

Broad clinical use would also depend on timely genetic diagnosis, referral before irreversible damage and consistent manufacturing.

Conclusion

The preliminary LIGHTHOUSE results give ATSN-201 a structural and functional signal that now requires confirmation in the pivotal Part C cohort using prespecified functional outcomes. Those data should show whether the therapy improves visual function enough to justify a one-time surgical procedure, and how narrow the optimal treatment window is. Genetic diagnosis, disease stage and surgical risk will define the treatable population.
