SpyGlass Pharma announced on October 1, 2026 that it has acquired Advanced Vision Science (AVS), its existing intraocular lens (IOL) manufacturer, for about $13 million. The deal gives SpyGlass direct control over a key component of its investigational BIM-IOL system, which combines an IOL with sustained delivery of bimatoprost and is in Phase 3 development. The efficacy data reported so far come from an earlier Phase 1/2 study and await confirmation in Phase 3.
The system pairs an IOL with sustained release of bimatoprost, a medication used to lower intraocular pressure (IOP). It is designed for patients undergoing cataract surgery who also require IOP management. Glaucoma is a chronic optic neuropathy in which elevated IOP is an important modifiable risk factor. Topical medications remain a major component of treatment, and consistent adherence is challenging for some patients. A drug-delivery implant reduces dependence on daily administration.
Company data previously reported for the higher-dose group:
These figures come from an early, selected cohort and were reported before the acquisition. They carry less weight than a controlled Phase 3 result and do not show that visual-field loss is prevented. Medication-free status also depends on how rescue criteria and follow-up were defined.
A drug-eluting lens is both an implant and a medicine-delivery platform, so consistency of lens manufacture and drug release must hold through scale-up, sterilization and routine cataract workflows. Owning AVS reduces one execution risk. The acquisition may also create opportunities to expand the range of IOL technologies compatible with the platform, and AVS’s work for other customers gives the deal a manufacturing-services dimension beyond BIM-IOL.
Long drug exposure inside an implanted lens raises practical questions about waning delivery, inflammation, lens clarity and the management of pressure that rises after the release period. The pivotal trial should report pressure control against an appropriate comparator, use of additional medication and adverse events over time. Regulatory review and long-term safety evaluation will also be required before widespread clinical adoption.
Owning AVS may reduce manufacturing risk for BIM-IOL, while the early pressure results remain unconfirmed by Phase 3. For surgeons and investors, the central question is whether a cataract-linked drug platform delivers reliable, sustained glaucoma control across ordinary patients after cataract surgery without excessive rescue treatment.
